You want to indicate whether you want to use your partner's surname or your own surname again. You submit this online with your DigiD to the municipality where you live.
What do you need to know
- At the municipality you indicate which last name the government (municipality, the Tax Authorities and the Social Insurance Bank) should use. You pass this on to other organisations, such as your bank and insurer
- If you get married or enter into a civil partnership, you choose which surname you want to use in the future. You indicate this at the notification of intended marriage or partnership and applies from the date of your marriage or registered partnership
- Your surname (given to you at birth) will not change. Only the way we address you in letters or emails. Official documents such as passport, identity card or driver's license always show your own surname (which you were given at birth).
- You indicate your choice for the use of your surname to the municipality where you live
Choice of last name
You can choose from 4 options for using a surname:
- Only your own last name
- Only your partner's last name
- Your own surname, followed by your partner's surname
- Your partner's last name, followed by your own last name
In case of divorce or death
If you are divorced or if your partner has died, you may continue to use your partner's last name. This is no longer allowed if you remarry or enter into a registered partnership.
